Thực trạng kiệt sức nghề nghiệp ở bác sỹ tại bệnh viện quận Thủ Đức, thành phố Hồ Chí Minh và một số yếu tố ảnh hưởng năm 2019

Tóm tắt

Mục tiêu: Mô tả thực trạng kiệt sức nghề nghiệp (KSNN) của nhân viên y tế bệnh viện quận Thủ Đức
năm 2019 và phân tích một số yếu tố ảnh hưởng đến kiệt sức nghề nghiệp của nhân viên y tế bệnh viện
quận Thủ Đức năm 2019.
Phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang, kết hợp phương pháp định luợng và định tính
được thực hiện tại bệnh viện quận Thủ Đức, thành phố Hồ Chí Minh, từ tháng 3 đến tháng 10 năm 2019.
Đối tượng nghiên cứu (ĐTNC): Bác sỹ, lãnh đạo bệnh viện, các quy định và chính sách hiện hành của
bệnh viện. Nghiên cứu sử dụng bộ câu hỏi đánh giá kiệt sức nghề nghiệp cho nhân viên y tế (Maslach
Burnout Inventory for Medical Personnel) gồm 22 câu hỏi.
Kết quả: Có 167 (41,4%) bác sỹ nam và 236 (58,6%) bác sỹ nữ. Điểm KSNN chung của bác sĩ là 53,0
± 23,5 (7-127) và cảm giác về hiệu quả chuyên môn của bản thân bác sỹ là 14,2 ± 8,1 (0-43). KSNN
chung được tìm thấy ở 62,6% bác sỹ. Ở từng khía cạnh KSNN, 73,4% bác sỹ bị kiệt sức về mặt tinh thần,
44,2% bác sỹ có cảm giác hoài nghi về bản thân và 64,8% số bác sỹ đối diện với KSNN khi cảm nhận
về thành tích bản thân. Các yếu tố về dịch vụ y tế, nhân lực, tài chính, cơ sở vật chất, điều hành và quản
lý đều đang ảnh hưởng theo hướng góp phần làm gia tăng KSNN ở các bác sỹ.
Kết luận: KSNN ở bác sỹ của bệnh viện quận Thủ Đức là khá cao. Các sức ép về khối lượng công việc/
dịch vụ y tế và con người, áp lực tự chủ tài chính, điều kiện cơ sở vật chất chật hẹp và yếu tố quản trị đã
và đang làm gia tăng KSNN ở bác sỹ.

Research methodology: cross sectional study is implemented at hospital of Thu Duc district from March to October, 2019. Research objects include physicians, hospital’s managers, an available hos- pital’s regulations and policies. Results: Prevalence of physicians’ burnout is high in all three main areas of symptoms which are 29,7% (Exhaustion); 12,2% (increased mental distance from one’s job) và 3% (reduced professional efficacy). Factors related services delivery, health workforce, finance, physical hospital environment, ledership and governance are contributing to increase burnout among physicians. Conclusion: Hospital’s managers need to review and/or revise current hospital’s human and financial regulations to minimise affeting factors toward burnout. Then, hospital environment needs to be renovated and upgraded friendly. Last, classes of coping stress in hospital are in need in order to prevent physicians’ burnout.
